Joining Lumina Family Care begins with selecting a participating health plan and choosing a primary care physician (PCP) who is affiliated with LFC.
How to Get Started
- Review the health plans currently contracted with Lumina Family Care.
- Contact the health plan to confirm your eligibility and available benefits.
- Select a LFC-affiliated primary care physician when enrolling or making a medical group selection.
- Confirm with your health plan that Lumina Family Care and your selected physician are shown as your assigned medical group and PCP.
Your health plan can assist you with enrollment, eligibility questions, PCP changes, and effective dates.
